What is the address where New York mails its extended unemployment job search list to?

I received a few sheets of paper (I think from the labor department) where I had to list my job searches for the month, and mail them in after the last sunday of the month. I mailed those sheets off last month, and do not have the address of where to send my job search to anymore. Does anyone have this information, or know where to get the address? Ive had a heluva time getting a hold of anyone at the Labor Department.|||it is the White House
